MAKE CONTACT:
It’s important to make contact with your team as early as possible after tryouts. The coach may need information sent out to get fall training started, and families also need to know who to contact with any questions that they have. Early contact helps to pull the team together.
- Get initial contact information (phone numbers/email addresses) from the coach or DSSC Registrar.
- Introduce yourself to your team by calling or sending an email.
- Request additional contact information from team families and create a team contact list. Information should include player full name, parents names, mailing address, home and cell phone numbers, and email addresses.
REQUIRED FORMS:
There are forms that need to be signed by players/parents and kept in your file that you keep for your team. The Medical Release and Liability forms must be carried with you to all games and tournaments. The forms can be accessed via the below links.
- WYSA MEDICAL RELEASE & LIABILITY FORM (18 & Older)
- WYSA MEDICAL RELEASE & LIABILITY FORM (Under 18)
- DSSC PHOTO RELEASE FORM
